Summary:
The Case Manager (CM) is responsible for implementing services specific to the program?s target population as required by the Scope of Work or other contract requirements.
The CM provides services of education and support to individuals and families with children ages birth to 5 years of age through home and/or community visitation.
The CM assesses the needs of the program?s target population then develops and implements a program plan specific to the identified needs of the target population.
CM activities include, but are not limited to, home visitation, education and support with health, nutrition, finances, parenting, early childhood development, referrals to other community agencies, follow-up, and data tracking.
The CM has the appropriate education, training, job experience, and/or equivalent experiences to operate independently and as a team member to carry out program activities with minimal supervision.

Estimated Salary: $20 to $28 per hour based on qualifications.
